Guest Vaccine and ID Submission

Chicago's Vaccine Mandate came into effect on 1/3/22. It requires guests aged 5+ to show proof of vaccination in order to attend private events. You can find the City of Chicago official guidelines here.

For guests ages 16+, please submit copies of both your Vaccination Card and valid Government Issued ID below. For attendees 5-15 years of age, a Vaccination Card is required however, no ID is required. Please submit each person’s proof separately.

We created this form to make things easier on you, as well as the hosts of the event. We ask that all attendees submit proof of vaccination and ID in advance, however, this is voluntary. Though it may not be necessary, all guests should be prepared to show their IDs and Vaccination Cards to security at the door. A photocopy or a clear photo of the Vaccination Card on your phone will suffice.

Privacy policy: No outside parties will have access to your information other than our staff and the event host. All data will be deleted immediately after the event.
